Navy: Gate security lapse led to fatal shooting aboard ship

A Navy investigation says security lapses at the main gate to the world's largest naval base allowed a civilian truck driver to come on base in 2014 and fatally shoot a sailor aboard a destroyer.

Petty Officer 2nd Class Mark Mayo was shot and killed aboard the USS Mahan at Naval Station Norfolk in Virginia. The report says Jeffrey Tyrone Savage drove on base without being asked for identification or his authorization to be there. The report says security at the gate failed to execute any standard operating procedures in response to an unauthorized person on base.

Savage was able to disarm one sailor standing watch on the Mahan and used her gun to shoot and kill Mayo, who rushed to her defense. Navy security shot and killed Savage.
